Summary

Off-pump coronary artery bypass surgery (OPCAB) benefits are debated. A local study found low event rates, making OPCAB superiority difficult to prove due to the large patient numbers required for statistical significance.

Area of Science:

  • Cardiovascular Surgery
  • Evidence-Based Medicine

Background:

  • The benefits of off-pump coronary artery bypass surgery (OPCAB) are frequently discussed in surgical literature.
  • Questions remain regarding the universal applicability and necessity of adopting OPCAB across all surgical practices.

Purpose of the Study:

  • To evaluate the feasibility of establishing the superiority of OPCAB in a local practice.
  • To determine the sample size required for a randomized controlled trial (RCT) to demonstrate OPCAB benefits.

Main Methods:

  • A retrospective observational study analyzed 507 patients who underwent coronary artery bypass grafting (CABG) with cardiopulmonary bypass.
  • Calculations were performed to estimate the patient cohort size needed for an RCT to detect significant improvements with OPCAB.

Main Results:

  • The study reported low event rates: mortality (1.4%), myocardial infarction (0.8%), renal dialysis (0.8%), and stroke (1.2%).
  • A significant number of patients (16%) required homologous blood transfusions.
  • An estimated 262,000 patients would be needed for an RCT to detect a 12.5% reduction in event rates, or 2300 patients for a 50% reduction.

Conclusions:

  • The local prevalence of adverse events is very low.
  • The sample size required for a definitive study on OPCAB superiority in this practice is prohibitively large.
  • The supremacy of OPCAB for this specific practice remains unestablished based on current data.
