Computerised Case-based testing (CCT) did not predict simulated patient (SP) performance but correlated with experience. SP scores were reliable but CCT may offer more valid performance assessment by focusing on efficiency.

Area of Science:

  • Medical Education
  • Performance Assessment
  • Rheumatology Training

Background:

  • Effective postgraduate medical training requires valid performance assessment tools.
  • Simulated patients (SPs) offer a method but are resource-intensive.
  • Computerised Case-based testing (CCT) presents a logistically simpler alternative.

Purpose of the Study:

  • To evaluate the predictive validity of CCT for assessing doctor performance.
  • To compare CCT with SP-based assessments in rheumatology training.

Main Methods:

  • Seventeen rheumatologists were assessed by eight SPs using a checklist.
  • A panel of experts identified essential checklist items.
  • Rheumatologists also completed a 55-case CCT with 121 items.

Main Results:

  • Unexpected negative correlations were found between SP and CCT scores.
  • CCT scores showed high positive correlations with experience.
  • SP scores showed high negative correlations with experience.

Conclusions:

  • CCT was not predictive of SP scores but correlated with experience.
  • SP scores may be less valid due to focus on thoroughness over efficiency.
  • Valid performance assessment prioritizes collected candidate information over method authenticity.
