Area of Science:

  • Urogynecology
  • Pelvic Floor Disorders
  • Surgical Outcomes

Background:

  • Burch colposuspension (Bc) is a surgical procedure for stress urinary incontinence.
  • Long-term effects of Bc on pelvic floor dysfunction (PFD) and bowel function require further investigation.

Purpose of the Study:

  • To assess the long-term prevalence of PFD concerning bowel function after Burch colposuspension (Bc).
  • To compare bowel function in women post-Bc with an age-matched general population sample.

Main Methods:

  • A follow-up study of 190 women who underwent Bc between 1980-1988.
  • Comparison with 305 age-matched women without anti-incontinence surgery.
  • Postal questionnaire assessing pelvic floor function in 1998, followed by univariate and multivariate analyses.

Related Experiment Videos

Main Results:

  • Women post-Bc reported significantly higher rates of bowel dysfunction compared to controls.
  • Specific issues included: manual assistance for defecation (OR 3.25), incomplete emptying (OR 2.29), lack of warning for defecation (OR 3.04), gas incontinence (OR 1.98), loose stool incontinence (OR 3.67), and daytime fecal leakage protection use (OR 3.22).
  • Bowel dysfunction adversely affected general well-being (OR 2.15).

Conclusions:

  • Burch colposuspension is associated with increased long-term pelvic floor dysfunction affecting bowel function.
  • These symptoms negatively impact patients' quality of life.
  • Multidisciplinary assessment and treatment for PFD are recommended.
