Summary

Barrett's esophagus (BE), a condition involving esophageal dysplasia, may be reversible in children with gastro-esophageal reflux (GER) following treatment. This study suggests BE is not rare in children and diagnosis requires biopsies.

Background:

  • Barrett's esophagus (BE) is characterized by esophageal dysplasia, where squamous mucosa is replaced by specialized columnar mucosa with goblet cells.
  • BE is an acquired condition often secondary to chronic acid exposure from gastro-esophageal reflux (GER).

Observation:

  • A review of pediatric BE cases and data from 19 handicapped children with GER was conducted.
  • Three out of 19 children with GER presented with intestinal metaplasia, indicative of BE.
  • Lower % time of esophageal pH < 4 was observed in patients with esophagitis, including BE, compared to those with normal esophagus.

Findings:

  • Esophageal pH monitoring revealed significantly reduced esophageal acid exposure time in children with esophagitis and BE.
  • BE in the studied pediatric population appeared to be reversible after surgical intervention and anti-acid therapy.

Implications:

  • Barrett's esophagus may not be a rare condition in the pediatric population, particularly in those with GER.
  • Biopsy specimens are crucial for establishing a definitive diagnosis of BE in children.
  • Early diagnosis and treatment of GER may lead to reversible BE in pediatric patients.
