Area of Science:

  • Obstetrics and Gynecology
  • Nephrology
  • Endocrinology

Background:

  • Preeclampsia is a serious pregnancy complication characterized by high blood pressure.
  • Hyponatremia, low sodium levels, can occur in pregnancy but is rare with preeclampsia.
  • Twin pregnancies are at higher risk for various complications.

Observation:

  • A 33-year-old woman with a twin pregnancy presented with mild preeclampsia.
  • She also exhibited unusual hyponatremia during the pregnancy.
  • The hyponatremia resolved quickly after childbirth.

Findings:

  • This represents the seventh reported instance of hyponatremia associated with preeclampsia.
  • Four of the seven patients, including this case, had twin pregnancies.

Related Experiment Videos

  • Four patients also presented with nephrotic syndrome.
  • Implications:

    • This case underscores the importance of monitoring electrolyte balance in pregnant patients with preeclampsia, especially those carrying multiples.
    • Further research is needed to understand the pathophysiology linking preeclampsia, hyponatremia, and twin gestations.
    • Early recognition and management of hyponatremia in preeclampsia may improve maternal and fetal outcomes.
