Summary

Cetuximab treatment can cause magnesium wasting and hypomagnesemia in cancer patients. Monitoring and magnesium supplementation are recommended to manage these electrolyte abnormalities.

Background:

  • Cetuximab, an EGFR inhibitor, is used for metastatic colorectal cancer.
  • Electrolyte disturbances, including hypomagnesemia, are potential side effects of cancer therapies.

Observation:

  • A patient on cetuximab developed severe hypomagnesemia and hypocalcemia.
  • Review of 154 patients revealed 22% had magnesium level checked, with 8 experiencing severe hypomagnesemia (grades 3-4).

Findings:

  • Cetuximab treatment is associated with magnesium wasting and hypomagnesemia.
  • EGFR expression in the kidney's loop of Henle suggests a mechanism for impaired magnesium reabsorption.

Implications:

  • Clinicians should monitor magnesium levels in patients receiving cetuximab.
