Summary

Routine upper gastrointestinal endoscopy in patients with abdominal aortic aneurysms (AAA) revealed a high prevalence of peptic ulcers. Early screening and surgery for gastric ulcers are recommended to reduce bleeding risks.

Background:

  • Abdominal aortic aneurysms (AAA) are associated with various comorbidities.
  • The prevalence and clinical significance of upper gastrointestinal (GI) peptic diseases in patients with intact AAA are not well-established.

Purpose of the Study:

  • To investigate the incidence of peptic diseases in Chinese patients with intact abdominal aortic aneurysms.
  • To assess the risk of GI bleeding associated with peptic ulcers in this patient cohort.

Main Methods:

  • Upper gastrointestinal endoscopy was performed in 106 patients with intact AAA.
  • Patients underwent screening or investigation for GI bleeding or pain.
  • Diagnoses included peptic ulcers (gastric and duodenal), gastritis, and duodenitis.

Main Results:

  • Peptic disease was found in 38 patients (35.8%).
  • Peptic ulcer disease was diagnosed in 26.4% of patients.
  • Gastric ulcers were more frequently associated with bleeding, particularly before aneurysmectomy (50% bled).
  • Duodenal ulcers had a higher risk of postoperative bleeding (16.7%).
  • Overall bleeding risk for ulcers associated with aneurysms was 35.7%.

Conclusions:

  • A significant proportion of patients with intact abdominal aortic aneurysms have peptic ulcer disease.
  • Routine endoscopic screening for peptic ulcers is suggested for patients with AAA.
  • Early surgical intervention for gastric ulcers may reduce the risk of potentially life-threatening bleeding.
