    Background:

    • Recurrent oral ulcers (ROU) are common, often idiopathic, and can significantly impact quality of life.
    • Childhood onset of ROU suggests a potentially chronic or systemic underlying etiology.
    • Previous exacerbation by orthodontic appliances warrants investigation into mechanical or inflammatory triggers.

    Observation:

    • A 33-year-old Caucasian female presents with a long-standing history of recurrent oral ulcers since childhood.
    • Ulcer duration was previously 7-10 days with recurrence every few months.
    • Current presentation involves persistent, intermittent irritation and ulceration, predominantly affecting bilateral buccal mucosa and lateral tongue.

    Findings:

    • The patient's oral ulcers have become more persistent and less predictable in their recurrence.
    • Despite removal of orthodontic braces, which previously exacerbated symptoms, no significant improvement in ulceration was noted.
    • The distribution of severe involvement to the buccal mucosa and lateral tongue may indicate specific patterns of mucosal vulnerability.

    Implications:

    • The chronicity and changing nature of these oral ulcers necessitate a thorough diagnostic workup to rule out underlying systemic conditions.
    • Further investigation into potential triggers, including autoimmune disorders or nutritional deficiencies, is warranted.
