Summary

A single preoperative dose of first-generation cephalosporins did not reduce infectious complications or hospital stay in women undergoing laparoscopy compared to no antibiotic prophylaxis. This prophylactic antibiotic regimen offers no added benefit.

Background:

  • Laparoscopy is a common gynecological surgical procedure.
  • Infectious morbidity is a potential complication following laparoscopic surgery.
  • Prophylactic antibiotics are often used to prevent surgical site infections.

Purpose of the Study:

  • To evaluate the effectiveness of a single preoperative dose of first-generation cephalosporins in reducing infectious morbidity in women undergoing laparoscopy.
  • To compare the incidence of postoperative infections between women receiving prophylactic antibiotics and a control group.

Main Methods:

  • A randomized, nonblinded comparative study involving 450 women undergoing laparoscopy.
  • Group 1 (n=200) received a single preoperative dose of 2g first-generation cephalosporins.
  • Group 2 (n=250) served as the control group without antibiotic administration.
  • Patients were monitored for 7 days post-surgery for signs of infection (wound, fever, peritonitis, UTI, respiratory).

Main Results:

  • No significant difference in the prevalence of postoperative infections between the cephalosporin group and the control group.
  • Mean hospital stay was not significantly different between the two groups.
  • No deaths were reported in either group.

Conclusions:

  • A single preoperative dose of 2g first-generation cephalosporins is clinically equivalent to no antibiotic prophylaxis for preventing infection in women undergoing laparoscopy in this setting.
  • The administration of this prophylactic antibiotic regimen offers no added benefit over the control group.
