Olfactory ensheathing cell (OEC) tumors can mimic schwannomas on the anterior skull base. Immunohistochemistry, particularly CD57 (Leu7) staining, is crucial for differentiating these rare tumors.

Background:

  • Schwannomas are common nerve sheath tumors, but their occurrence on the anterior skull base is rare.
  • Olfactory ensheathing cells (OECs) share similarities with Schwann cells but have distinct immunohistochemical profiles.

Observation:

  • A 31-year-old woman presented with a convulsion and an anterior skull base tumor.
  • Radiological and histological examination revealed a heterogeneously enhanced, spindle-shaped cell tumor.
  • Initial suspicion of schwannoma was revised based on S-100 positivity and negativity for EMA and CD57 (Leu7).

Findings:

  • The tumor exhibited immunohistochemical characteristics consistent with an olfactory ensheathing cell (OEC) tumor.
  • OEC tumors are differentiated from schwannomas by their CD57 (Leu7) negativity.
