Summary

Portal vein embolization (PVE) with absolute ethanol effectively induced liver regeneration in pigs. This study observed a 14.2% increase in the future liver remnant to total estimated liver volume ratio post-procedure.

Background:

  • Portal vein embolization (PVE) is a critical pre-operative procedure to induce hypertrophy of the future liver remnant (FLR).
  • Absolute ethanol is a commonly used embolic agent, but its efficacy in PVE requires further evaluation in preclinical models.
  • Multidetector-row computed tomography (CT) angiography offers detailed visualization for assessing PVE outcomes.

Purpose of the Study:

  • To assess the effectiveness of percutaneous transhepatic portal vein embolization (PVE) using absolute ethanol.
  • To evaluate liver regeneration and histopathologic changes following PVE in a porcine model.
  • To correlate volumetric changes with histological findings after PVE.

Main Methods:

  • Percutaneous transhepatic PVE with 10 ml absolute ethanol was performed on liver segments/subsegments in 10 pigs.
  • CT angiography and volumetric analysis were used to measure FLR hypertrophy and total estimated liver volume (TELV).
  • Histopathologic examination was conducted 2-6 weeks post-PVE to assess liver tissue changes.

Main Results:

  • Successful vessel occlusion was achieved immediately post-ethanol injection (0.25-0.33 ml/kg).
  • The mean FLR/TELV ratio increased by 14.2% after PVE, indicating significant liver regeneration.
  • Histological analysis confirmed liver atrophy in embolized areas, becoming distinct by 3 weeks post-PVE.

Conclusions:

  • Portal vein embolization with absolute ethanol successfully stimulated a regenerative response in the porcine liver.
  • Histopathologic changes, including atrophy, were observed in the embolized liver segments.
  • The study demonstrated a mean FLR/TELV ratio increase of 14.2% following PVE with absolute ethanol.
