Summary

A symptomatic intraorbital arteriovenous malformation was successfully treated with surgery. Complete removal of the vascular malformation was confirmed, resolving eye chemosis.

Background:

  • Intraorbital arteriovenous malformations (AVMs) are rare vascular anomalies that can present with varied symptoms.
  • Symptomatic presentation without exophthalmos is uncommon for intraorbital AVMs.

Observation:

  • A patient presented with left eye chemosis and bruit, indicative of a symptomatic intraorbital arteriovenous malformation.
  • Exophthalmos was notably absent despite the symptomatic nature of the orbital vascular anomaly.

Findings:

  • Surgical excision via a fronto-orbital approach achieved complete removal of the intraorbital AVM.
  • Postoperative imaging (MRI and angiography) confirmed successful resection and resolution of the vascular malformation.
  • Histopathology revealed the AVM was confined to extrabulbar fatty tissue, without invading critical orbital structures like extraocular muscles or the optic nerve.
