Summary

The standard partogram for labor monitoring does not accurately reflect the faster progression of multigravid (previous birth) labors. A customized partogram is needed to better represent the unique characteristics of subsequent pregnancies.

Area of Science:

  • Obstetrics and Gynecology
  • Maternal-Fetal Medicine

Background:

  • Current labor management relies on partograms based on primigravid (first birth) labor patterns.
  • Multigravid labor is recognized as progressing faster than primigravid labor.
  • The 'usual-care' partogram is applied to both primigravid and multigravid labors.

Purpose of the Study:

  • To evaluate the suitability of the standard 'usual-care' partogram for multigravid spontaneous labor.
  • To develop and test a customized partogram reflecting the accelerated cervical dilation in multigravid labors.

Main Methods:

  • Audit of 1398 partograms from spontaneous multigravid labors resulting in vaginal delivery.
  • Retrospective graphing of labor data on both 'usual-care' and customized partograms.
  • Development of a customized alert line representing the cervical dilation rate of the lower 90% of multigravid labors.

Related Experiment Videos

Main Results:

  • The 'usual-care' partogram's norms do not accurately represent the progression of multigravid labor.
  • A customized partogram, with an alert line based on 90% of multigravid labor rates, provides a more fitting representation.
  • Testing different action line placements (2, 3, or 4 hours behind alert line) showed varied distributions.

Conclusions:

  • The standard partogram is inadequate for monitoring multigravid labor.
  • A customized partogram is necessary to accurately reflect the faster cervical dilation in subsequent labors.
  • Clinical practice should consider adapting labor monitoring tools for multigravid women.
