Summary
Urinary bladder matrix (UBM) patches promote cardiac tissue regeneration after heart attack, showing potential for myocardial repair. UBM biodegraded and was replaced by contractile cells, outperforming synthetic ePTFE patches.
Area of Science:
- Regenerative Medicine
- Biomaterials Science
- Cardiovascular Research
Background:
- Heart failure is a major health concern.
- Tissue-engineered cardiac patches offer a promising therapeutic strategy.
- Extracellular matrix scaffolds are being investigated for myocardial repair.
Purpose of the Study:
- To evaluate urinary bladder matrix (UBM) as a scaffold for repairing infarcted left ventricles.
- To compare UBM with expanded polytetrafluoroethlyene (ePTFE) for cardiac patching.
- To assess the tissue response and regenerative potential of UBM in a porcine model.
Main Methods:
- Pigs (n=42) underwent left ventricular (LV) infarction.
- 4-layer multilaminate UBM or ePTFE patches were implanted as full-thickness LV wall replacements.
- Tissue analysis included histology, immunocytochemistry, and flow cytometry at 1 week, 1 month, and 3 months post-implantation.
Main Results:
- At 3 months, UBM was bioresorbed, replaced by vascularized, fibrocellular tissue with myofibroblasts and some contractile cells.
- UBM showed significantly more alpha-smooth muscle actin-positive cells compared to ePTFE at 1 month (P=0.04).
- ePTFE patches exhibited foreign-body response, necrosis, and calcification at 3 months, with limited cardiomyocyte markers.
Conclusions:
- Biodegradation of UBM was accompanied by the formation of fibrocellular tissue containing contractile cells.
- UBM demonstrates superiority over synthetic ePTFE for cardiac patching.
- UBM shows a trend towards myocardial replacement at 3 months post-implantation.

Overview
In order to maintain tissue organization, many animal cells are surrounded by structural molecules that make up the extracellular matrix (ECM). Together, the molecules in the ECM maintain the structural integrity of tissue as well as the remarkable specific properties of certain tissues.

Overview of Cell-Matrix Interactions
8.0K
The extracellular matrix or ECM holds cells together to form a tissue and allows the cells within the tissue to communicate. ECM comprises proteins such as fibronectin, collagen, laminin, etc. The most abundant protein in this space is collagen. Collagen fibers are interwoven with carbohydrate-containing protein molecules called proteoglycans. ECM allows cell migration and provides a structural scaffold at cell adhesion that anchors the cell when the extracellular matrix proteins interact with...

Cell-matrix's Response to Mechanical Forces
2.7K
In animal cells, the extracellular matrix allows cells within tissues to withstand external stresses and transmits signals from the outside of the cell to the inside. The extracellular matrix is extensive, and its composition varies between different types of tissues. For example, the reticular fibers and ground substance make up the ECM in loose connective tissue, while collagen and bone minerals make up the ECM of bone tissue.

Extracellular Matrix
6.0K
Unlike epithelial tissue, which is composed of cells closely packed with little or no extracellular space in between, connective tissue cells are dispersed in a matrix. This extracellular matrix (ECM) is composed of fibrous proteins like collagen, elastin, and fibronectin in a ground substance consisting of interstitial fluid, cell adhesion proteins, and proteoglycans. The proteoglycans form a gel-like material in the spaces between cells and provide hydration, buffering, binding, and force...
