Summary

This study found no significant difference in serum antibody responses to periodontal pathogens between localized aggressive periodontitis (LAP) and generalized aggressive periodontitis (GAP) patients. Infection patterns do not distinguish between aggressive and chronic periodontitis classifications.

Background:

  • The current classification of periodontitis distinguishes between localized aggressive periodontitis (LAP) and generalized aggressive periodontitis (GAP).
  • A prevailing postulate suggests LAP patients exhibit robust serum antibody (ab) responses to periodontal pathogens, while GAP patients show weak responses.
  • Antibody responses in localized chronic (LCP) and generalized chronic periodontitis (GCP) were also investigated.

  • To investigate the validity of the postulate differentiating LAP and GAP patients based on serum antibody responses to periodontal pathogens.

  • Plaque samples were analyzed for 15 bacterial species, and serum immunoglobulin G (IgG) responses were determined.
  • Antibody responses were quantified using log-transformed titres and "infection ratios".

Main Results:

  • The study failed to corroborate the postulate of robust antibody responses in LAP versus weak responses in GAP patients.
  • No significant differences in antibody titres or infection ratios were observed for "red complex", "orange complex", or health-associated species.

Conclusions:

  • The study's findings indicate that infection patterns do not support a distinction between the principal categories of aggressive and chronic periodontitis.
  • Current periodontitis classification may not be reliably differentiated by analyzing serum antibody responses to specific periodontal pathogens.
