Summary

Ileitis in ulcerative colitis (UC) is common after restorative proctocolectomy and indicates a distinct patient subset with a poorer prognosis. Routine ileal examination is crucial for understanding UC outcomes.

Area of Science:

  • Gastroenterology
  • Colorectal Surgery
  • Pathology

Background:

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
  • Restorative proctocolectomy is a surgical option for UC.
  • Ileitis, or inflammation of the ileum, can occur in UC patients.

Purpose of the Study:

  • To determine the incidence, demography, pathology, and clinical significance of ileitis in UC patients post-restorative proctocolectomy.
  • To compare patients with and without ileitis regarding clinical and pathological characteristics.
  • To assess the impact of ileitis on the development of pouchitis.

Main Methods:

  • Review of a prospectively collected pouch database and case notes of 100 UC patients undergoing restorative proctocolectomy (1988-2003).

Related Experiment Videos

  • Blind re-examination and grading of proctocolectomy specimens and pouch biopsies.
  • Comparison of demographic, clinical, and pathological characteristics between patients with and without ileitis.
  • Main Results:

    • Ileitis was present in 22% of patients.
    • Patients with ileitis had shorter disease duration, a higher risk of fulminant disease, and strong associations with pancolitis and primary sclerosing cholangitis.
    • Ileitis significantly increased the incidence of subsequent pouchitis.

    Conclusions:

    • Ileitis in UC is not rare and negatively impacts prognosis, challenging the term "backwash ileitis".
    • UC with ileitis represents a distinct disease subset.
    • Routine microscopic examination of the terminal ileum and prospective outcome audits are essential for accurate assessment.
