High-lethality suicide attempts in Borderline Personality Disorder (BPD) are predicted by low socioeconomic status (SES) and comorbid disorders, not BPD severity. These factors identify individuals at higher risk for completed suicide.
Area of Science:
- Psychiatry
- Clinical Psychology
- Suicidology
Background:
- Recurrent suicidal behaviors in Borderline Personality Disorder (BPD) are often viewed as communicative, yet suicide completion remains a significant risk (10%).
- Identifying specific risk factors for suicide lethality within BPD is crucial for targeted intervention and prevention strategies.
Purpose of the Study:
- To identify predictors differentiating high-lethality from low-lethality suicide attempts in patients diagnosed with BPD.
- To compare demographic, diagnostic, and historical variables between BPD patients with varying suicide attempt lethality.
Main Methods:
- A comparative study of 113 BPD attempters, divided into High-Lethality (n=44) and Low-Lethality (n=69) groups based on Beck's Medical Lethality Scale.
- Multivariate logistic regression analysis was employed to determine predictors of high-lethality status using assessed variables.
Main Results:
- High-lethality attempters were older, had children, lower education, and lower socioeconomic status (SES).
- They were more likely to have comorbid Major Depressive Disorder (MDD), Antisocial Personality Disorder (ASPD), family substance abuse history, greater intent to die, and more prior hospitalizations.
- Key predictors for high-lethality status included low SES, comorbid ASPD, extensive treatment history, and higher intent to die.
Conclusions:
- Risk factors for high-lethality suicide attempts in BPD, such as low SES and comorbid ASPD, mirror profiles of completed suicides.
- These predictors are not specific to BPD and do not appear to be linked to impulsivity, aggression, or the severity of BPD criteria.
