Summary

Endodermal sinus tumors (ESTs) are rare pediatric malignancies. This case highlights successful treatment of a rare vaginal EST in an infant using radical surgery and chemotherapy, with normalized alpha-fetoprotein (AFP) levels.

Background:

  • Malignant germ cell tumors (GCTs) represent less than 3% of pediatric malignancies.
  • Endodermal sinus tumor (EST) is the most common GCT histological subtype.
  • Vaginal GCTs are exceptionally rare in pediatric patients.

Observation:

  • A one-year-old female presented with vaginal bleeding, pallor, and a palpable pelvic mass.
  • MRI revealed a tumor at the cervix-vagina junction; biopsy confirmed vaginal EST.
  • Elevated serum alpha-fetoprotein (AFP) levels were noted.

Findings:

  • The patient underwent successful in toto vaginohysterectomy followed by four cycles of cisplatin, etoposide, and bleomycin chemotherapy.
  • At one-year follow-up, the patient was symptom-free with normalized AFP levels.
