Summary

Diagnosing vascular dementia remains challenging. Current clinical criteria are not interchangeable and have limitations in sensitivity and specificity, impacting the accurate identification of mixed dementia cases.

Area of Science:

  • Neurology
  • Geriatrics
  • Neuroscience

Background:

  • Vascular dementia (VaD) diagnosis has been challenging for over a century.
  • Standardized diagnostic criteria for VaD have been proposed but show variability.
  • Existing criteria exhibit limitations in accurately differentiating VaD subtypes and mixed dementia.

Purpose of the Study:

  • To evaluate the performance of current clinical criteria for diagnosing vascular dementia.
  • To assess the interchangeability and accuracy of different diagnostic criteria sets.
  • To highlight the need for improved diagnostic criteria for vascular and mixed dementia.

Main Methods:

  • Review of published clinical criteria for vascular dementia.
  • Analysis of clinical studies comparing different diagnostic criteria sets.
  • Retrospective clinicopathological correlation studies.

Main Results:

  • Clinical criteria for vascular dementia identify distinct patient groups and are not interchangeable.
  • Most criteria demonstrate insufficient sensitivity but acceptable specificity.
  • Criteria exclude pure Alzheimer's disease but may misclassify 9-39% of mixed dementia cases.

Conclusions:

  • Current diagnostic criteria for vascular dementia require improvement.
  • Further research is needed to develop more accurate and sensitive criteria.
  • A consensus on improved criteria is essential for accurate diagnosis of vascular and mixed dementia.
