Non-dilated obstructive nephropathy, a rare condition, often delays diagnosis of acute renal failure. Prompt relief of urinary tract obstruction rapidly restores kidney function in these cases.

Area of Science:

  • Nephrology
  • Urology
  • Radiology

Background:

  • Obstructive uropathy typically presents with urinary tract dilatation.
  • Non-dilated obstructive nephropathy is an uncommon variant.

Observation:

  • Described are five cases of non-dilated obstructive nephropathy in uraemic patients.
  • Causes included retroperitoneal malignancy and uric acid lithiasis.
  • Standard imaging (ultrasound, CT, renography) failed to suggest obstruction.

Findings:

  • Radionuclide scans showed poor perfusion and parenchymal phase images.
  • Relief of obstruction led to immediate diuresis and rapid renal function recovery.

Implications:

  • The absence of dilatation can lead to delayed diagnosis of this treatable cause of acute renal failure.
