  • A 16-year-old female presented with progressive gait difficulty 8 years post-spinal radiation for astrocytoma.
  • MRI revealed intramedullary multicentric cavities in the thoracic spine (T4-10).

Findings:

  • Subtotal resection of the lesion was performed.
  • Pathological examination confirmed cavernous malformation with radiation-induced vascular degeneration.
  • This represents the third reported case of de novo intramedullary cavernous malformation after spinal radiation.
