Summary

This study reports a rare case of short QT interval (SQTI) syndrome in a young male presenting with exertional dyspnea. Despite the high risk for sudden cardiac death, the patient remained asymptomatic and was managed with medication.

Background:

  • Short QT interval syndrome (SQTS) is a rare genetic disorder characterized by a markedly shortened QT interval on electrocardiogram.
  • It is associated with an increased risk of ventricular arrhythmias and sudden cardiac death.
  • Diagnosis often requires a combination of ECG findings, electrophysiologic studies, and genetic testing.

Observation:

  • A 20-year-old male presented with exertional dyspnea, exhibiting fine atrial fibrillation and a short QT interval (<300 ms) on ECG.
  • Echocardiography showed an interatrial septal aneurysm and a slightly dilated pulmonary artery.
  • Electrophysiologic study revealed rapid atrial fibrillation, a short ventricular effective refractory period (130 ms), and inducible ventricular fibrillation.

Findings:

  • The patient's clinical presentation and diagnostic findings were consistent with the rare short QT interval syndrome.
