Summary

Endoscopic retrograde cholangiopancreatography (ERCP) may lead to long-term biliary bacteriology changes. Biliary bacteriology associated with bile duct stones persists even after endoscopic treatment.

Background:

  • Biliary bacteriology is crucial in hepatobiliary diseases.
  • Endoscopic retrograde cholangiopancreatography (ERCP) is a common diagnostic and therapeutic procedure for biliary disorders.

Purpose of the Study:

  • To investigate the impact of ERCP on biliary bacteriology and cytology.
  • To assess the persistence of biliary bacteriology in patients with and without bile duct stones.

Main Methods:

  • Prospective study of 32 patients undergoing two ERCP procedures.
  • Bile samples collected during ERCP for bacteriology and cytology analysis.
  • Patients categorized into Group I (normal bile ducts) and Group II (bile duct stones with sphincterotomy).

  • Bactibilia was significantly more prevalent in patients with gallstones (19/23) compared to those with normal ERCP (0/9).
  • In Group I, bactibilia increased from 27% initially to 60% at follow-up.

Conclusions:

  • ERCP may lead to a sustained increase in biliary bacteriology, suggesting procedural contamination.
  • Biliary bacteriology associated with bile duct stones is persistent and not resolved by endoscopic treatment.
  • No clear correlation was found between biliary cytology and bacteriology.
