Crohn's disease can cause anal incontinence, particularly with severe symptoms or diarrhea. However, anorectal function remains normal in Crohn's patients without these issues.

Background:

  • Crohn's disease frequently affects the perianal region.
  • Anal incontinence is a significant complication impacting quality of life.

Purpose of the Study:

  • To evaluate anorectal function in patients with Crohn's disease.
  • To identify factors associated with anal incontinence in this population.

Main Methods:

  • Anorectal manometry was performed on 63 Crohn's disease patients and 10 controls.
  • Anal sphincter pressures, rectal capacity, and incontinence symptoms were assessed.

Main Results:

  • 17% of Crohn's patients had partial and 5% had total anal incontinence.
  • Incontinent patients exhibited significantly lower maximal basal and squeeze pressures.
  • Severe anorectal stenosis and prior anal abscess surgery were risk factors for total incontinence; diarrhea was linked to partial incontinence.

Conclusions:

  • Crohn's disease with severe anorectal issues or diarrhea significantly impairs anorectal function.
  • Crohn's disease without macroscopic anorectal lesions or diarrhea does not affect anorectal function.
