Background:

  • Spinal epidural hematoma (SEH) is uncommon in children.
  • Idiopathic SEH, without a clear cause, is even rarer.
  • This report focuses on a unique case of recurrent idiopathic SEH in a pediatric patient.

Observation:

  • A 6-year-old girl presented with acute thoracic back pain, neck stiffness, and gait disturbance.
  • Initial spinal MRI revealed an idiopathic SEH compressing the spinal cord from T1 to T3.
  • The hematoma resolved spontaneously over 22 days, with the patient becoming asymptomatic.

Findings:

  • The patient experienced a sudden recurrence of SEH 59 days after initial presentation, with worsening neurological symptoms.
  • Repeat MRI confirmed the reappearance of the hematoma at the same spinal level.
