    Background:

    • Recurrent herpes simplex type 1 (HST1) infections are known to cause neuralgia and chronic facial pain.
    • Chronic neuralgia following a single HST1 infection, simulating postherpetic neuralgia, has not been previously documented.

    Observation:

    • A 49-year-old woman presented with oral-facial dyskinesia, followed by burning pain and hypersensitivity in the oral-facial region, then a vesicular rash confirmed as HST1.
    • Despite initial treatment with acyclovir and subsequent trials of NSAIDs and opioids, the patient experienced persistent hyperesthesia, allodynia, and burning pain.

    Findings:

    • The patient's persistent neuropathic pain, including hyperesthesia and allodynia, resolved after initiating amitriptyline (75 mg/day).
    • Discontinuation of amitriptyline led to pain recurrence, with symptoms controlled upon restarting the medication.
    • Diagnostic tests including MRI, EEG, and evoked potentials were normal, ruling out other neurological causes.

    Implications:

    • This case highlights that neuralgia following HST1 infection can clinically mimic postherpetic neuralgia.
    • Amitriptyline demonstrates potential efficacy in managing chronic neuropathic pain associated with HST1 infections.
    • Early recognition and appropriate treatment, such as with amitriptyline, are crucial for managing post-HST1 neuralgia.
