Area of Science:

  • Infectious Diseases
  • Travel Medicine
  • Public Health

Context:

  • Traveller's diarrhea (TD) is the most common health concern for international tourists.
  • Existing preventive measures like pretravel advice and probiotics lack consistent efficacy.
  • Vaccination provides only limited and short-term protection against TD.

Purpose:

  • To review the prophylactic and therapeutic strategies for Traveller's diarrhea.
  • To identify effective treatments and recommendations for different traveler groups.
  • To inform travel health guidelines regarding TD management.

Summary:

  • Antibiotic chemoprophylaxis is highly effective, reducing TD incidence by 80-90%, but is reserved for specific high-risk individuals.
  • Standby treatment, including rehydration, loperamide, and fluoroquinolones, is advised for travelers visiting most high-risk destinations.
  • Neither general pretravel advice nor probiotic use has demonstrated reliable prophylactic benefits against TD.

Impact:

  • Provides evidence-based recommendations for managing Traveller's diarrhea in clinical practice.
  • Aims to reduce the incidence and impact of TD on global travelers.
  • Highlights the importance of risk assessment for antibiotic prophylaxis in travel medicine.
