Background:

  • Idiopathic anaphylaxis (IA) presents a diagnostic and therapeutic challenge.
  • Severe cases of IA, termed malignant IA, are resistant to standard therapies.
  • Northwestern University's Division of Allergy and Immunology (NUAI) has extensive experience with IA patients.

Observation:

  • Over 16 years, 225 IA patients were evaluated and treated.
  • Four patients were identified with malignant IA.
  • Malignant IA is defined by the inability to reduce oral corticosteroid doses without exacerbation.

Findings:

  • Long-term evaluation of one patient with malignant IA is presented.
  • Three additional patients with malignant IA were managed.
  • No deaths due to IA occurred in patients treated with the described pharmacologic regimen.

  • This study highlights the characteristics and management of severe, corticosteroid-resistant IA.
  • The findings suggest that pharmacologic management can be effective in preventing fatal outcomes in malignant IA.
