Summary

Perifolliculitis capitis abscedens et suffodiens (PCAS) is a rare, chronic scalp condition. A 29-year-old male experienced excellent results with a long course of isotretinoin treatment for PCAS.

Background:

  • Perifolliculitis capitis abscedens et suffodiens (PCAS) is a rare, chronic, suppurative, and inflammatory scalp disease.
  • Its etiology and pathogenesis remain incompletely understood, posing treatment challenges.

Observation:

  • A 29-year-old male presented with a 3-year history of tender, fluctuant scalp nodules, abscesses with draining pus, and patchy alopecia.
  • Initial treatment with incision and drainage yielded poor results.

Findings:

  • Skin biopsy confirmed features characteristic of perifolliculitis.
  • Isotretinoin, initiated at 30 mg daily and later reduced to 10 mg due to mild hyperlipidemia, resulted in an excellent and rapid response.

Implications:

  • PCAS treatment is often difficult and frustrating for both patients and physicians.
