Summary

A chronic cough in a 72-year-old man was linked to ventricular premature contractions (VPCs). Treating the VPCs with disopyramide resolved both the cough and palpitations, suggesting a hemodynamic cause.

Area of Science:

  • Cardiology
  • Pulmonology
  • Clinical Medicine

Background:

  • Chronic cough is a common clinical complaint.
  • Ventricular premature contractions (VPCs) can cause various symptoms.
  • The relationship between cardiac arrhythmias and cough is not fully understood.

Observation:

  • A 72-year-old male presented with chronic cough associated with palpitations.
  • Electrocardiography (ECG) showed normal sinus rhythm with sporadic unifocal VPCs.
  • Each cough episode was preceded by a premature beat.

Findings:

  • Echocardiography revealed a transient increase in pulmonary artery blood flow induced by VPCs.
  • Treatment with oral disopyramide successfully resolved both cough and palpitations.
  • This suggests a link between VPC-induced hemodynamic changes and cough.

Related Experiment Videos

Implications:

  • VPC-induced hemodynamic changes in pulmonary circulation may cause coughing.
  • Consider premature contractions as a potential cause of chronic dry cough.
  • This case highlights the importance of a thorough cardiac evaluation in unexplained chronic cough.
