Summary

Neurologic toxicity from amiodarone can cause debilitating ataxia. Discontinuing this medication, even at low doses, led to rapid and complete symptom recovery in an elderly patient.

Background:

  • Amiodarone is an antiarrhythmic drug with known potential for neurologic toxicity.
  • Neurologic side effects, including ataxia, are under-recognized consequences of amiodarone therapy.

Observation:

  • An elderly woman with hypertrophic obstructive cardiomyopathy and atrial fibrillation developed progressively debilitating ataxia.
  • The patient was receiving a small dose of amiodarone.

Findings:

  • Discontinuation of amiodarone resulted in rapid and complete remission of ataxia.
  • Neurologic symptoms improved within days, with full recovery of ambulation within a week, despite amiodarone's long half-life.

Implications:

  • Early recognition of amiodarone-induced neurologic toxicity can prevent extensive diagnostic workups.
