  • Temporal arteritis (giant cell arteritis) is an inflammatory condition affecting large arteries.
  • Bilateral oculomotor nerve palsy is an exceptionally rare presentation of temporal arteritis.

Observation:

  • A 65-year-old male presented with subacute, pupil-sparing bilateral third nerve palsy.
  • This presentation prompted an investigation for underlying temporal arteritis.

Findings:

  • Temporal artery biopsy results confirmed the diagnosis of temporal arteritis.
  • The patient showed significant improvement following corticosteroid therapy.

Implications:

  • Temporal arteritis should be considered in the differential diagnosis of ophthalmoplegia in elderly patients.
