Summary

Nasal packing with cotton wool for epistaxis (nosebleeds) can lead to foreign body granulomas if not fully removed. Prompt identification and removal of any prior nasal packing are crucial for effective epistaxis management.

Area of Science:

  • Otolaryngology
  • Emergency Medicine
  • Pathology

Background:

  • Epistaxis (nosebleeds) is a common emergency presentation.
  • Nasal packing is a standard treatment for epistaxis.
  • Foreign bodies in the nasal cavity can cause complications.

Observation:

  • A patient presenting with epistaxis had used cotton wool for initial nasal packing.
  • This cotton wool was potentially pushed further back during subsequent medical packing.
  • The retained foreign body resulted in the formation of a foreign body granuloma.

Findings:

  • Foreign body granuloma formation in the nasal cavity due to retained cotton wool packing.
  • Difficulty in nasal examination and foreign body retrieval during active epistaxis.

Related Experiment Videos

  • The importance of inquiring about and removing prior nasal packing before definitive treatment.
  • Implications:

    • Highlights the risk of retained foreign bodies from self-administered nasal packing.
    • Emphasizes the need for thorough examination and history taking in epistaxis management.
    • Proposes the "epistaxis management ladder" to standardize and improve trainee understanding of treatment protocols.
