Summary

Sarcoidosis can cause massive splenomegaly, a rare condition. This case highlights the importance of considering sarcoidosis in the differential diagnosis of enlarged spleens.

Observation:

  • A patient presented with massive splenomegaly, initially suspected to be lymphoma.
  • Surgical exploration revealed sarcoidosis with significant splenic involvement.

  • The spleen weighed 2250g, representing one of the largest recorded cases of splenic sarcoidosis.
  • While splenic involvement in sarcoidosis is common, massive splenomegaly is rare, occurring in only 3% of documented cases.

Implications:

  • Sarcoidosis should be included in the differential diagnosis for patients presenting with splenomegaly.
