Background:

  • Chronic combat-induced post-traumatic stress disorder (PTSD) presents treatment challenges.
  • Many patients with PTSD show inadequate response to standard psychotropic agents.
  • Atypical antipsychotics have shown some efficacy in PTSD treatment.

Observation:

  • Two male patients with chronic PTSD, without active psychosis, were treated with ziprasidone.
  • Both patients had a history of insufficient response to multiple prior psychotropic medications.
  • Ziprasidone was selected for its broad neurotransmitter activity.

Findings:

  • Both patients experienced significant benefit from ziprasidone therapy.
  • Ziprasidone was effective in managing chronic PTSD symptoms in these cases.

  • Implications:

    • This is the first report on ziprasidone's utility in PTSD pharmacotherapy.
    • Ziprasidone may be a viable treatment option for PTSD patients refractory to other agents.
