Summary

A new grading scale for hip arthritis predicts outcomes for total hip resurfacing. Hips with fewer unfavorable characteristics (Grades A/B) showed better results, indicating strict patient selection improves success rates.

Purpose of the Study:

  • To evaluate the association between a novel arthritic hip grading scale and short-term outcomes of metal-on-metal total hip resurfacing.
  • To determine if strict patient selection based on proximal femur characteristics improves THR success.

  • Retrospective prognostic study (Level II evidence).
  • 147 consecutive hips underwent metal-on-metal resurfacing by a single surgeon.
  • Hips were graded A-F based on four proximal femur characteristics; 91 hips had a minimum 2-year follow-up.

Conclusions:

  • Lesser secondary arthritic changes correlate with higher arthritic hip grades and better THR outcomes.
  • Strict selection criteria for hip resurfacing are associated with a low incidence of short-term failures.
