Subclinical cardiovascular issues and cardiac dysfunction increase with longer type 1 diabetes duration. Reduced flow-mediated dilation (FMD) is linked to abnormal heart function in these patients.

- Diastolic function was assessed using the Em/Am ratio.
- Laboratory analyses and stepwise regression were performed.

- Type 1 DM patients showed reduced FMD and increased IMT compared to controls.
- Abnormalities in regional diastolic and systolic function were detected early in the disease.
- Reduced FMD was the strongest predictor of abnormal segmental cardiac function.
Conclusions:
- Subclinical cardiac dysfunction and cardiovascular risk increase with type 1 DM duration.
- Flow-mediated dilation is significantly associated with abnormal segmental cardiac function in type 1 DM.
- These findings highlight the importance of monitoring cardiovascular health in type 1 DM patients.
