Summary

This study found that water fluoridation at 1 ppm was associated with a small but significant increase in hip fracture risk for elderly men and women. These findings suggest a potential link between low-level fluoride exposure and increased fracture incidence in older populations.

Background:

  • Water fluoridation is a common public health measure for dental health.
  • The effects of water fluoridation on bone health, particularly hip fractures in the elderly, require further investigation.

Purpose of the Study:

  • To evaluate the association between community water fluoridation at 1 part per million (ppm) and the incidence of hip fractures in individuals aged 65 and older.

Main Methods:

  • An ecological cohort study design was employed.
  • Compared hip fracture incidence rates in three Utah communities: one with fluoridated water (1 ppm) and two without.
  • Analyzed hospital discharge data for first-time hip fractures in individuals 65+ over a 7-year period, excluding specific cases.

Main Results:

  • A relative risk of 1.27 for hip fractures was observed in women residing in the fluoridated area compared to non-fluoridated areas (95% CI = 1.08 to 1.46).
  • A relative risk of 1.41 for hip fractures was observed in men residing in the fluoridated area compared to non-fluoridated areas (95% CI = 1.00 to 1.81).

Conclusions:

  • The study identified a statistically significant increase in hip fracture risk for both men and women exposed to 1 ppm of fluoride in community water.
  • These findings suggest that low levels of fluoride exposure through water may be associated with an elevated risk of hip fractures in the elderly population.
