Summary

Obese pregnant women face higher risks, including pre-eclampsia and difficult labor. Proactive epidural analgesia can reduce maternal mortality and complications for these high-risk patients.

Area of Science:

  • Obstetrics and Gynecology
  • Anesthesiology
  • Public Health

Background:

  • Rising global obesity rates present significant challenges in obstetric care.
  • Obese parturients experience increased risks of medical comorbidities and antenatal complications like pre-eclampsia and gestational diabetes.
  • Obesity is a critical risk factor for anesthesia-related maternal mortality.

Purpose of the Study:

  • To highlight the increased risks associated with obesity in pregnancy.
  • To advocate for proactive anesthetic management, including consultation and regional techniques, for obese parturients.
  • To emphasize strategies for reducing anesthesia-related maternal mortality in this population.

Main Methods:

  • Review of existing literature on obesity in pregnancy and anesthesia.
  • Analysis of risks and complications in obese parturients.
  • Discussion of anesthetic techniques and their implications.

Main Results:

  • Obese women have a higher incidence of abnormal labor, instrumental delivery, and Cesarean sections.
  • Anesthetic management of obese parturients is challenging but crucial for patient safety.
  • Regional anesthesia techniques, particularly epidural analgesia, can be beneficial when used judiciously.

Conclusions:

  • Morbidly obese women are high-risk and require specialized antenatal anesthetic consultation.
  • Liberalized use of regional anesthesia, including prophylactic epidural catheter placement, may decrease maternal and perinatal complications.
  • Effective postoperative care, including early mobilization and pain control, is vital for obese parturients.
