Intra-thyroid thyroglossal duct cysts are rare in children. This case highlights the diagnosis and surgical management of an intra-thyroid thyroglossal duct cyst, emphasizing fine needle aspiration cytology for diagnosis.

Area of Science:

  • Pediatric Surgery
  • Endocrinology
  • Head and Neck Surgery

Background:

  • Thyroglossal duct cysts (TDCs) are congenital anomalies typically found in the midline of the neck.
  • Intra-thyroidal presentation of TDCs is exceptionally rare, particularly in the pediatric population.

Observation:

  • An 11-year-old patient presented with a palpable neck mass on the right thyroid lobe.
  • Sonography revealed a 2 cm unilocular cyst within the thyroid gland.
  • A gammagram showed no radioisotope uptake (cold lesion), and fine needle aspiration cytology (FNAC) suggested TDC.

Findings:

  • Surgical exploration found no thyroglossal tract, leading to simple cyst enucleation.
  • Pathological examination confirmed an intra-thyroidal thyroglossal duct cyst.

Related Experiment Videos

  • The patient remained recurrence-free with no complications after 8 months of follow-up.
  • Implications:

    • This case adds to the scarce literature on pediatric intra-thyroidal TDCs.
    • Diagnosis relies heavily on FNAC, as imaging may not reveal the tract.
    • Surgical management involves careful exploration for the tract, though its absence is noted.
