Area of Science:

  • Thoracic surgery
  • Gastrointestinal surgery
  • Critical care medicine

Background:

  • Chylothorax is the accumulation of chylous lymphatic fluid in the pleural space, often caused by thoracic duct injury or obstruction.
  • While cancer is the most common cause, iatrogenic factors and trauma also contribute to chylothorax.
  • Post-operative complications following gastric surgery can include rare but serious conditions like chylothorax.

Observation:

  • A 76-year-old woman developed bilateral chylothorax, predominantly on the right side, in the post-operative period after gastric surgery.
  • The patient presented with respiratory insufficiency and tachycardia, but no immediate hemodynamic instability.
  • The condition manifested during the recovery phase following surgical intervention.

Findings:

Related Experiment Videos

  • The case highlights a rare instance of bilateral chylothorax occurring after gastric surgery.
  • Conservative treatment, involving the cessation of oral intake and initiation of total parenteral nutrition (TPN), was implemented.
  • Chest tube drainage was utilized for a period of 10 to 14 days to manage the effusion.

Implications:

  • This case underscores the importance of recognizing chylothorax as a potential post-gastric surgery complication.
  • Conservative management, including nutritional support and pleural drainage, can be effective in resolving post-operative chylothorax.
  • Early diagnosis and appropriate management are crucial for favorable outcomes in patients with chylothorax.
