Background:

  • Clavulanic acid is frequently co-administered with beta-lactam antibiotics to combat bacterial resistance.
  • Adverse reactions to clavulanic acid are uncommon, with IgE-mediated responses being rare due to its low allergenicity.

Observation:

  • A young adult developed a delayed, generalized itchy erythema two days post-amoxicillin-clavulanate therapy.
  • Previous antibiotic courses with clavulanic acid were tolerated, and immediate hypersensitivity tests were negative.

Findings:

  • Skin testing with amoxicillin-clavulanate elicited a delayed reaction at 24 and 48 hours.
  • The reaction pattern suggests a non-immediate, possibly T-cell mediated hypersensitivity to clavulanic acid.
