Summary
Pulmonary neuroendocrine cell hyperplasia shows high epidermal growth factor receptor (EGFR) expression, unlike most neuroendocrine lung tumors. This suggests EGFR may play a role in hyperplasia development and offers a potential therapeutic target.

Background:

Purpose of the Study:
- To investigate the expression levels of EGFR in pulmonary neuroendocrine cell hyperplasia and different types of neuroendocrine pulmonary tumors.

Main Methods:
- Immunohistochemical analysis was performed on 15 cases of pulmonary neuroendocrine cell hyperplasia (carcinoid-tumorlets, DIPNECH) and 20 neuroendocrine pulmonary tumors (carcinoid tumors, large cell neuroendocrine carcinomas, small cell neuroendocrine lung carcinomas).
- EGFR (HER-1) expression was quantified and characterized by staining patterns (membranous, cytoplasmic).
Main Results:
- All cases of neuroendocrine cell hyperplasia demonstrated maximum EGFR expression (score 3 in 100% of cells) with predominantly membranous and partly cytoplasmic staining.
- Four out of ten carcinoid tumors were strongly positive for EGFR, while the remaining six were negative.
- A significant majority (90%) of large cell neuroendocrine and small cell neuroendocrine carcinomas were negative for EGFR.
Conclusions:
- Overexpression of EGFR in pulmonary neuroendocrine cell hyperplasia suggests its potential significance in the pathogenesis of these lesions.
- Given the clinical manifestations of DIPNECH, targeting EGFR/HER-1 or its downstream signaling pathways could be a promising therapeutic strategy.
