Summary

This study investigated Kaposi sarcoma in six patients, suggesting a vascular endothelial cell origin. Findings indicate growth factor regulation in Kaposi sarcoma development, challenging its rarity in Okinawa.

Area of Science:

  • Oncology
  • Pathology
  • Vascular Biology

Background:

  • Six Kaposi sarcoma cases observed in Okinawa, previously considered rare.
  • Patient cases included classic type, association with adult T-cell leukemia (ATL), multiple myeloma, and acquired immune deficiency syndrome (AIDS).
  • Lesion presentation varied from nodular skin lesions to plaques on trunk, face, and extremities, with some involving lymph nodes and perineural tissues.

Purpose of the Study:

  • To investigate the cellular origin and growth mechanisms of Kaposi sarcoma.
  • To characterize the immunohistochemical and histochemical features of Kaposi sarcoma in diverse patient groups.

Main Methods:

  • Immunohistochemical and lectin histochemical analyses on tumor tissue sections.
  • Cell culture of tumor mass for proliferation and characterization studies.
  • Electron microscopy to examine cellular structures.

Main Results:

  • Positive staining for Factor VIII-related antigen, UEA-1, GS-1 lectins, and EGF receptor in tumor cells, suggesting endothelial origin.
  • Absence of common viral antigens (CMV, HBV, HSV, HPV, HIV) in most cases, except CMV adjacent to tumor in one AIDS patient.
  • Cultured spindle cells exhibited proliferation and formed tube-like structures, positive for Factor VIII-related antigen, EGF receptor, UEA-1, and GS-1.

Conclusions:

  • Kaposi sarcoma cases were diagnosed in a short period, indicating it may not be as rare as previously thought in Okinawa.
  • Evidence suggests Kaposi sarcoma originates from vascular endothelial cells.
  • Tumor growth appears to be regulated by growth factors.
