GIST: a clinical dilemma.

Summary

This case study highlights a rare malignant gastric and duodenojejunal gastrointestinal stromal tumor (GIST). Despite high malignancy, the patient experienced an 8-year disease-free survival post-surgery without chemotherapy, suggesting lifelong surveillance for GIST patients.

Background:

Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumors of the gastrointestinal tract.
Malignant GISTs, particularly those with high mitotic rates, typically require adjuvant therapy post-resection.
The behavior of GISTs can be unpredictable, even with benign histological features.

Observation:

A 69-year-old female presented with a malignant gastric and duodenojejunal GIST.
The patient underwent surgical resection as the sole treatment, refusing chemotherapy.
Histological analysis revealed high-grade spindle cell tumors (>20 mitoses/10 HPF).

Findings:

The patient achieved an 8-year disease-free survival with no evidence of recurrence or metastasis.

This outcome occurred despite the high malignancy grade and absence of adjuvant chemotherapy.

The case demonstrates a potential for long-term survival in select high-risk GIST cases after complete surgical resection.

Implications:

Lifelong follow-up is proposed for all patients undergoing potentially curative surgery for GIST, regardless of initial risk stratification.
This approach accounts for the unpredictable behavior of GIST and the potential for late recurrence.
The findings challenge current adjuvant therapy guidelines for certain high-risk GIST cases.
