Intercellular adhesion molecule-1 (ICAM-1) expression in the uterine lining does not change during the menstrual cycle or with long-term progestin-only contraceptive use. This suggests other factors influence neutrophil influx into the endometrium.

Background:

  • Neutrophils accumulate in the endometrium during the premenstrual phase and with long-term progestin-only contraceptive (LTPOC) use.
  • Neutrophil migration is mediated by intercellular adhesion molecule-1 (ICAM-1) on endothelial cells.
  • Previous research on ICAM-1 expression in the endometrium yielded conflicting results regarding cell type specificity and cyclic variation.

Purpose of the Study:

  • To investigate the expression patterns of ICAM-1 in the endometrium across the menstrual cycle and during LTPOC treatment.
  • To determine if ICAM-1 expression by endothelial cells contributes to neutrophil infiltration in the endometrium.

Main Methods:

  • Endometrial biopsies were collected from women throughout their menstrual cycles and from those using LTPOCs (Mirena, Norplant).

    • Glands showed no ICAM-1 staining, while stromal cells exhibited weak positive staining.

    Conclusions:

    • Altered endothelial ICAM-1 expression does not appear to be the cause of neutrophil infiltration in the premenstrual or LTPOC-treated endometrium.
    • The influx of neutrophils into the endometrium may be regulated by changes in neutrophil chemoattractant expression.
