Background:

Isolated infundibuloarterial inversion (IAI) is a rare conotruncal anomaly.
IAI typically presents with severe associated cardiac defects.
Ventriculoarterial concordance is usually present in IAI.

Observation:

This report details a case of IAI with a small, hemodynamically insignificant ventricular septal defect.
The aortic valve was noted to be leftward and posterior.
The condition was distinguished from anatomically corrected malposition of the great arteries.

Findings:

IAI can occur in isolation or with minimal additional defects.
Diagnostic imaging modalities are crucial for identifying IAI.
