Background:

  • Monochorionic monoamniotic (MoMo) twins represent a rare pregnancy type (1 in 10,000).
  • MoMo twins are associated with significant risks including cord entanglement, congenital malformations, twin-to-twin transfusion syndrome (TTS), and prematurity, leading to high perinatal morbidity and mortality.

Purpose of the Study:

  • To review the clinical experience with 36 sets of MoMo twins from 1990 to 2005.
  • To provide updated data for improved patient counseling regarding MoMo twin pregnancies.

Main Methods:

  • Chorionicity was confirmed via placental examination.
  • Gestational age and TTS were assessed clinically and via sonography.
  • Intrauterine growth restriction (IUGR) was diagnosed using a twin-specific nomogram.

Main Results:

  • Cord entanglement occurred in 15 pregnancies with minimal morbidity. Four of 71 live births exhibited IUGR. Congenital malformations were diagnosed in 5 fetuses/infants. TTS affected 3 sets of twins.
  • Deliveries occurred between 31 and 34 weeks gestation for most sets (31/36). Overall mortality was 8% (6/71 twins), primarily due to malformations, TTS, or extreme prematurity.
  • Cranial ultrasounds in survivors revealed low rates of periventricular leukomalacia (3%) and intraventricular hemorrhage (9%), with most (88%) showing normal findings.

Conclusions:

  • Monochorionic monoamniotic twins continue to be a high-risk group requiring vigilant monitoring for cord entanglement, malformations, TTS, and prematurity.
  • Despite the inherent risks, the neonatal mortality and morbidity associated with MoMo twins appear to be lower than previously anticipated, suggesting improved outcomes.
