Summary

Primary bone lymphomas are rare, mostly non-Hodgkin lymphoma (NHL). Diffuse large B-cell lymphoma (DLBCL) is common, and patients often have a favorable outcome with combined therapy.

Area of Science:

  • Hematology
  • Oncology
  • Pathology

Background:

  • Primary lymphomas of bone are uncommon malignancies, predominantly non-Hodgkin lymphoma (NHL).
  • Primary Hodgkin lymphoma (HL) of bone is exceptionally rare.
  • Patients typically present with bone pain, swelling, and pathological fractures, often over 45 years old with a male predominance.

Purpose of the Study:

  • To review the histogenesis of primary non-Hodgkin lymphoma (NHL) of bone.
  • To correlate findings with the recent histogenetic classification of diffuse large B-cell lymphoma (DLBCL).
  • To understand the B-cell differentiation origins of these rare bone tumors.

Main Methods:

  • Comprehensive immunohistochemical studies are essential for accurate diagnosis.
  • Classification follows the World Health Organization (WHO) system for hematological malignancies.

Related Experiment Videos

  • Staging utilizes the Ann Arbor system, with most cases being stage IE or IIE.
  • Main Results:

    • Diffuse large B-cell lymphoma (DLBCL) is the most frequent type of primary NHL of bone.
    • Combined modality therapy is associated with a favorable outcome.
    • Clinical stage is a significant prognostic variable for overall survival.

    Conclusions:

    • Primary NHL of bone, particularly DLBCL, may originate from post-germinal center B-cells.
    • Understanding B-cell differentiation pathways is crucial for elucidating the origins of these lymphomas.
    • Further research into histogenesis can refine diagnostic and therapeutic strategies.
