Background:

  • Panniculitic T-cell lymphoma (PTCL) is a rare and aggressive cutaneous lymphoma.
  • Diagnostic challenges arise from its mimicry of benign panniculitis, both clinically and histologically.

Observation:

  • A 34-year-old male presented with a thigh plaque, fever, and constitutional symptoms, initially misdiagnosed as cellulitis.
  • Lack of improvement with antibiotics prompted further investigation.

Findings:

  • Two skin biopsies confirmed panniculitic T-cell lymphoma.
  • The patient received 8 cycles of CHOP chemotherapy.

Implications:

  • This case highlights the importance of considering PTCL in persistent, non-responsive skin lesions.
