Background:

Panniculitic T-cell lymphoma (PTCL) is a rare and aggressive cutaneous lymphoma.
Diagnostic challenges arise from its mimicry of benign panniculitis, both clinically and histologically.

Observation:

A 34-year-old male presented with a thigh plaque, fever, and constitutional symptoms, initially misdiagnosed as cellulitis.
Lack of improvement with antibiotics prompted further investigation.

Findings:

Two skin biopsies confirmed panniculitic T-cell lymphoma.
The patient received 8 cycles of CHOP chemotherapy.

Implications:

This case highlights the importance of considering PTCL in persistent, non-responsive skin lesions.
